The application must be in the name of the organization that holds the 501(c)(3) classification by the IRS.
Example:
ABC Family Resource Center would like funding for operating expenses. It operates very much like an independent organization. It has its own director, advisory board, and does its own fundraising. However, the ABC Family Resource Center does not have its own 501(c)(3) classification. It operates under the umbrella of ACME Nonprofit and is included in ACMEs financial audit. In this case, the Applicant will be ACME Nonprofit.
Indicate whether this grant is for a particular program or project, or if it is for general operating support. Your request should be consistent with your telephone interview with the Program Officer.
Indicate the total amount of the grant you are requesting. If this is a multi-year grant request, indicate whether it is two-year request or a three-year request. Include the amount being requested per year. Your request should be consistent with the amount and timeframe you discussed with the Program Officer.
NOTE:If your request has changed since your conversation with the Program Officer, follow-up contact is necessary.
Your organization must be classified as a 501(c)(3) to be eligible for funding unless it is a branch of state or county government.
What is your organization’s mission and what services do you provide to accomplish this mission?
List your organization’s main sources of financial support. It is helpful if you include the dollar amount or indicate the percentage this support represents of your organization’s total operating budget. Include any significant foundation grants or United Way funding you receive. It is also helpful if you indicate the total of your organization’s annual expense budget.
Be as specific as possible indicating the program activities and budget items for which the funds will be spent.
Remember, the Foundation typically does not support such items as administrative overhead, advertising, community education, seminars, food and refreshments, incentives, handbook or manual publication, staff training, "scholarships" or direct payments to or for individuals.
